Please fill in application completely, accurately and legibly!

Application must be signed by Team Official to be considered for JMD.

Read JMD Tournament Rules to clarify Team / Team Officials responsibilities.

AWARDS: 1ST and 2ND Place awards
Tournament pins and programs for all players.

FORMAT: US Youth Soccer and FIFA rules

U/9-U/10 play modified rules (8 v 8)

U/9 -U/14 play 25 minute halves

U/15-U/19 play 30 minute halves

All finals will be regulation time per age group

Guest Players: Teams will be allowed 5 guest players

Credentials: All players must have 2009/2010 US Youth Soccer player passes, Medical release forms,

copy of birth certificate and travel papers.

NO PASS OR PAPERWORK, NO PLAY.

Acceptance Letters: Acceptance letters will be emailed.

Referees: We need Referees for the tournament. If interested, please email JammersRus@aol.com or

Check in: Mandatory team check-in on Friday night before the tournament
